HomeRelay turns a hospital discharge packet into a safe, shared first 72 hours at home. In the demo, Robert is discharged with a 17-page packet, multiple medication instructions, a missing follow-up, and family tasks that still need owners. His daughter Maya uses HomeRelay to convert those instructions into a coordinated home plan with medications, follow-ups, caregiver responsibilities, safety review, and a simple Today view for Robert. Band is the coordination layer behind the workflow, not a notification add-on. HomeRelay uses Band for separate agent identities, shared mission rooms, dynamic specialist recruitment, structured task assignment, private clinical handoff rooms, agent responses, human escalation evidence, and room/event history. This allows multiple agents to collaborate through planning, execution, review, decision-making, and task handoff. AI/ML API is used where model capability matters: discharge packet interpretation and structured extraction, medication reconciliation assistance, follow-up coordination assistance, and independent safety review through a separate reviewer path. The event trace exposes model work, tool calls, fallback recovery, and final review evidence. HomeRelay does not replace clinicians or make medical decisions. It coordinates the handoffs families are usually left to manage alone.
