NFO Clinical AI is a multi-agent clinical decision support platform built for regulated and high-stakes healthcare workflows. Developed for the Band of Agents Hackathon Track 3, the system demonstrates how specialized AI agents can collaborate while maintaining transparency, traceability, and physician oversight. The platform uses six agents working together through a coordinated workflow. The Symptom Extractor structures clinical information from patient inputs. The Diagnostic Reasoner generates differential diagnoses and identifies missing evidence. The SOAP Writer produces structured clinical documentation. The Management Planner provides physician-facing recommendations with safety guardrails. The Patient Instructions agent converts findings into understandable guidance. Finally, the Clinical Reviewer performs a safety review, flags inconsistencies, and provides feedback before outputs are finalized. To improve reliability, NFO Clinical AI combines LLM reasoning with deterministic clinical scoring systems including HEART, Wells, qSOFA, BE-FAST, CHA₂DS₂-VASc, CURB-65, and Alvarado. This augments AI reasoning with established evidence-based risk assessment methods. A key feature is FactTrace, an evidence-tracing system that records every clinical fact used during reasoning. Diagnoses, recommendations, and reports can be traced back to symptoms, vital signs, and medical history, creating an auditable workflow suitable for regulated environments. The platform also supports emergency detection for high-risk presentations such as acute coronary syndrome, stroke, and sepsis. PDF report generation, JSON export, reviewer feedback loops, and Band-powered collaboration timelines provide visibility into how agents communicate and contribute throughout the workflow.
